We are seeking a talented Program Manager for our Population Health group.  In this key position, you will be focused on value-based care contracts.
You will be responsible for providing actionable insights and managing key projects to support Interwell Health business goals and strategy, create and manage long term goals, and participate in continuous improvement of project management best practices, processes and tools.   

The work you will do:

  • Track team projects to ensure goals are achieved to effectively manage our total population.  
  • Cross collaborate with external teams on Population Health projects. 
  • Tracks and monitors progress and escalates business risks to Senior Director where appropriate. 
  • Participate in population health improvement opportunities. Measure the progress of improvement projects by analyzing data. 
  • Deliver actionable insights to Senior Director on cross collaboration projects.  

 The skills and qualifications you need:

  • Bachelor’s Degree in business or related field 
  • 2-4 years of program or project management experience in value-based payer programs (required)
  • Proven track record in delivery of small to medium-scale projects and programs   
  • Proficient in interpreting and analyzing data 
  • Advanced proficiency in Excel
  • Excellent ability to communicate ideas clearly and persuasively
  • Excellent presentation skills
