As a Program Manager within Interwell Health’s Provider Operations Team, your key responsibility will be to act as a liaison between our provider network and nephrology practices, facilitating a smooth transition to Value-Based Care. This role requires a diligent work ethic, excellent communication skills, and the ability to work independently and collaboratively with external partners and internal, cross-functional teams.  

The work you will do: 

  • Understanding the objectives and complexities of intricate Value-Based Care Programs and/or agreements, such as:
    • CMS Kidney Care Contracting  
    • IWH Payor Agreements
    • IWH Practice Participation Agreements Serving as primary contact and subject matter expert for IWH Network practices. 
  • Assisting our nephrology partners in achieving success within value-based care by uncovering valuable insights within data and identifying opportunities for improvement. 
  • Executing company initiatives and ensuring the timely completion of deliverables. 
  • Ensuring accurate documentation of provider interactions/visits is properly maintained.  
  • Delivering timely training and/or onboarding to Network Providers.  
  • Verifying that information presented to providers is both accurate and timely, including the data accessible through the Provider Portal 

The skills and qualifications you will need: 

  • Bachelor’s degree required; Advanced Degree desirable 
  • Experience working with physicians and practices, nephrology-specific experience a plus 
  • Travel Requirements – Must be willing to travel 50% of the time to practices in the Northeast and MidAtlantic region.
  • Superior attention to detail and exceptional follow-through
  • Should possess an independent and proactive approach, showcasing dedication to continuous learning
  • Excellent communication and interpersonal skills to effectively collaborate with providers, stakeholders, and internal teams.
