About the Role
In this pivotal role, you won’t just be part of the care team—you’ll be at its core, ensuring that every patient’s experience is as seamless, precise, and impactful as possible. Working side by side with a physician, health coach, and patient success coordinator, you’ll help create a level of care that feels as personal as it is powerful.
Your work is the foundation that allows physicians to focus on what they do best—making the most insightful and impactful medical decisions. You will take complex patient information and transform it into clear, actionable insights that guide care plans. But your role isn’t just about organizing data; it’s about shaping the patient journey. You’ll be actively involved in facilitating and refining medical plans, ensuring that each decision is backed by precision and purpose.
Beyond patient care, you’ll be the essential link between the physician, health coach, and patient success coordinator, helping to integrate knowledge across the team. Your insights will not only enhance how we support patients but will also empower your teammates to work at their highest level. The result? A deeply collaborative approach that directly improves patient outcomes and extends the healthy, active years in their lives.
This is more than a role—it’s an opportunity to be part of something transformative. As we build and refine this innovative new program, you’ll play a key role in shaping workflows, creating SOPs, and streamlining processes that make patient care more effective. Just as we encourage our patients to pursue continuous improvement, we apply the same mindset to our own work. We believe that great ideas can come from anywhere, and we welcome fresh perspectives, creative solutions, and a spirit of continuous refinement.

- Turning complex patient data into meaningful insights – You don’t just gather information; you make sense of it. Your ability to extract key patterns and translate data into actionable next steps helps ensure that every patient receives truly personalized, high-quality care.
- Crafting clinical documentation that is both thorough and efficient – You take pride in capturing every essential detail while keeping documentation clear, concise, and easy to navigate, ensuring that physicians can quickly access what they need, when they need it.
- Preparing in-depth analyses that empower physicians – You anticipate what will be most valuable, distilling vast amounts of information into well-organized, insightful reports that enable smarter, faster medical decisions.
- Creating patient communications that are clear, supportive, and actionable – You recognize that a well-worded message can make all the difference in a patient’s confidence and adherence to their care plan. Your ability to communicate with empathy and clarity helps patients feel truly supported on their health journey.
- Working seamlessly with the care team to execute complex medical protocols – You are the steady, detail-oriented problem solver who keeps everything running smoothly. Whether coordinating next steps, ensuring follow-through, or catching the small things others might miss, you bring a level of reliability that your team and patients can trust.
